Community Needs Survey

Terms and Conditions

For your organization to be eligible for the $500 charitable donation, your completed survey must be submitted no later than June 25, 2024. The winning organization will be selected at random. Odds of winning are dependent on the number of completed surveys received. For a survey to be considered complete, you must provide your organization’s name, first name, last name, and valid email address. Only one entry per person. An organization can have multiple entries. Winning organization will be announced on our website by July 8, 2024 and contacted directly via email.


The winner will be notified by email no later than July 8, 2024. Once notified, the winner must provide an invoice and their organization’s W9 within 60 calendar days. If an invoice and valid W9 are not received within 60 days, the initial winner forfeits the charitable donation. A new winner will be randomly selected, and the process will begin again. Once the required documents are provided, Beneficial State Bank will issue a check or ACH payment within 60 calendar days to the organization (not the individual). Beneficial State Bank and Beneficial State Foundation are not eligible.
